New Patient Registration

Patients wishing to register with this practice will be asked to bring details of their previous practice. You will then be asked to complete a registration form and if possible provide proof of identity (passport or driving licence). An appointment will then be made for you to see a nurse for a health check. Having had the health check, you will then be fully registered and able to consult with a doctor.

At the point of registration, you will be encouraged to sign up and use our on line patient services, like making and cancelling your appointments, ordering your repeat prescriptions etc.

When you register at Newtown Surgery, you are registering as a patient of the practice, rather than with a specific doctor. Your named GP is Dr Koya. If you wish, you may express a preference to see a specific doctor and we will try to accommodate your wish. However this may not always be possible but we can assure you that, with excellent liaison between our clinical staff and comprehensive computerised medical records, any of the doctors will be able to help you when you visit the surgery.

Disabled Patient Facilities

Disabled patient facilities are also available at this practice.

Change of Personal Details

Once registered at the surgery, should you change your name, address or telephone number at any time, please let us know by contact reception or writing to the surgery. It is also helpful to let us know a change of mobile number as soon as possible. If you move outside the practice area you will be asked to find another doctor nearer to where you are living.

Guide to GP Services

The Royal College of General Practitioners has produced a useful guide for patients about the services on offer at GP Surgeries and how to access them.  You can download the guide below.
